RCT(ROOT CANAL TREATMENT)
Indications
RCT is done when
- There is decay in the tooth & has reached to the level of nerve in the tooth.
- There is injury to the teeth and nerve is exposed.
- There is severe sensitivity to hot and cold due to natural wear of enamel and dentin.
- Teeth are to be reduced for putting a bridge (in select cases).
- There is infection beneath the root of the tooth causing swelling & infection.
Procedure
Is done under local anesthesia. Removal of the decayed tooth part is done with the dental drill to gain access to the nerve. The infected nerve is removed with help of sterile needles(files). Proper cleaning and shaping of root canals is done to make sure no infection is left. Sealing of root canals is done with gutta percha points to prevent future infection. Finally the tooth cavity is filled up with filling material.Complete procedure takes 3-4 sittings on alternate days.
SINGLE SITTING RCT
In select cases RCT can be done in one sitting of around 45-60 minutes & antibiotic coverage is given for 3-5 days. Mild pain killers may me needed.
*all root canal treated teeth are to be followed by a crown(cap) mandatorily.

CROWNS (CAPS)
Crowns more commonly known as caps are covering on the tooth similar to shape & size of normal tooth.
INDICATIONS
Crowns are indicated in
- Teeth which are root canal treated.
- Teeth broken due to injury.
- Highly discolored teeth due to fluorosis,any systemic problem,or due to external factors like excessive pan chewing.
- Correction of shape and size of natural teeth.

BRIDGES
These are fixed teeth made using support of adjoining teeth and are cemented on to these teeth permanently. Bridges can be usedto replace small no. of missing teeth/tooth provided adjoining teeth have sufficient strength(bone support).
Types of material used for crowns and bridges can be
- METAL - Nickel Chromium Alloy is used and is steel colored in appearance.
- PRECIOUS METAL - Pure Gold can also be used as a metal for crown.
- PORCELIN WITH METAL (PFM) - These are tooth coloured crowns made up of Porcelin(Ceramic) with underlying base of metal.
- FULL CERAMIC - These are ceramic crown without any metal base underlying so are more aesthetic and natural like.
FILLINGS
Those dark colored silver fillings considered best till recent past are now rarely used because of,
- The unaesthetic colour of the filling.
- The mercury content of the amalgam.

COLORED FILLINGS

PIT AND FISSURE SEALENTS
These are similar to dental cements but are for prophylactic use. Thus they make these grooves smooth and allow proper cleaning and so they reduce the threat of decay to a great extent.
ORTHODONTICS

PROCEDURE
Placing implants is a very simple but technic sensitive procedure. Usually done under local anesthesia can be
- Two stage surgery with delayed function
- One stage surgery with delayed function
- Immediate function
As with many dental procedure implant placement also needs antibiotics and mild pain killers post operatively.you are back to work the very next day without any hospitalization.
In two stage surgery around three months after placing the implant it is opened up and crown or bridge is fixed.
ORAL SURGERY
- IMPACTED MOLAR - Most common surgical procedure in routine dental practice is removal of an impacted third molar or commonly known as wisdom tooth.In majority of people due to lack of space in arch thirdmolar are either not present or if present are partially/completely impacted into the bone.
Why remove impacted wisdom tooth?
If totally embedded in the bone & are not causing any problem to the patient there is no need to remove these teeth as long as they are lying innocently but they are to be removed if,
- It causes recurrent pain &/or swelling.
- It is partially erupted and is difficult to clean,causes food impaction & decay in adjoining tooth.
- Causes gum inflammation and foul smeel.
- It pinches cheek while eating causing permanent ulceration & pain in the cheek.
PROCEDURE - Involves incising the gums above the impacted tooth & then removing the bone which covers the tooth .once the tooth is free of locking it is removed.stitches are taken to approximate the gums.these stitches are removed after a week.
Slight pain,swelling and discomfort is obvious and it remains from 2-5 days depending upon individual tolerance and body response.But after that its trouble free for life.
It is advisable to get the wisdom tooth in opposing arch also removed at the same time.
- PERIAPICAL CYST - Usually occours in relation to the front upper teeth due to injury in childhood and patient not getting any treatment at that time
Symptoms
- The affected tooth gets dark in colour and turns blackish/bluish
- There is off & on pus discharge above the root of the tooth/teeth involved.
- Some times a huge swelling occours above the tooth which was asymptomatic for years.
- Patient has sensitivity to hot and cold and x-ray reveals a cyst or chronic abscess.
PROCEDURE - Involves giving a incision on the root area of affected teeth & removing the cyst or abscess ,cleaning the area thoroughly and root canal treatment of affected tooth/teeth in same sitting itself.sutures are taken.
*the affectd teeth need crown later on to mask the discoloration.
GUM TREATMENT(PERIODONTAL TREATMENT)
Gum diseases is a major cause of about 70% of tooth loss.three fourth of adult Indian population is suffering from one or other periodontal problem.
